(We have removed the PMD Face to Face Examination Guide as some physicians were completing this Guide as a stand alone document rather than documenting information in their patient's chart from the required face-to-face examination of the patient. In its place we have created the PMD Medicare Order and Documentation Requirements Quick Reference Tool.)
A reference tool for physicians to assist with completing the necessary information on the prescription for a Power Mobility Device as well as documenting the necessary information from the mandatory face-to-face examination of the patient.
- Choosing The Right Power Mobility Solution For Your Patient:
Provides guidance in determining which Power Mobility Device is appropriate based on a patient's condition, an assessment of the patient's living environment, and functionality of the mobility device.
- Power Mobility Device Prescription:
A sample prescription form that contains all the necessary elements that are required on a prescription per the Interim Final Rule/Local Coverage Determination policy.
- Medicare Physician Resource Guide:
Provides guidance to physicians on the current coverage policy for PMDs, face-to-face exam requirements, PMD prescription requirements, and what to include in supporting documentation/informations.

A form for Providers to utilize to obtain important information from the patient/caregiver specific to the patient's home environment to assist in determining the most suitable PMD for the patient.
