    A reference tool for physicians to assist with completing the necessary information on the prescription for a Power Mobility Device as well as documenting the necessary information from the mandatory face-to-face examination of the patient.
     
  • Choosing The Right Power Mobility Solution For Your Patient:

    Provides guidance in determining which Power Mobility Device is appropriate based on a patient's condition, an assessment of the patient's living environment, and functionality of the mobility device.
     
  • Power Mobility Device Prescription:

    A sample prescription form that contains all the necessary elements that are required on a prescription per the Interim Final Rule/Local Coverage Determination policy.
