Literature and Literature Shipment Pricing Policy

Effective July 23, 2007

Dear Pride Provider,

To allow us to better focus our resources into the development of the innovative products, strong sales programs, our extensive Government Affairs efforts, and comprehensive reimbursement services needed to grow and enhance your business, we are announcing a restructuring of some of our Provider Marketing Services policies.

As you know, Pride’s Provider Marketing Services put the power of an award-winning, in-house marketing agency at your disposal. We remain fully committed to delivering high quality marketing pieces that support your marketing efforts at minimal cost.

Effective July 23rd, a new system will be implemented based on your year to date sales. Here is how the new policy will work:

• You will be allotted a pre-determined amount of free general product support per year. (Please contact your accounts executive for amounts)
• Pride’s Literature Department will track all your requests and inform you when you’ve reached your maximum of free general product support.
• Shipping and handling charges will still apply on all orders.
• After the free amount of literature has been filled, all providers will be charged the standard cost per piece for literature (please see our standard literature costs below).
• The policy on new product sell sheets still applies. New product sell sheets will be available at no cost for 3 months. Providers are eligible for up to 25 pieces of the new product sell sheet in this period.

The cost of non-custom literature will be as follows:

• Sell sheets, Brochures and End to Ends: Each specific brochure will be distributed in packets of 10. Packets of 10 pieces $1.20 per pack
• Posters: $1.00 each
• Ad slicks, hangtags, and stuffers: 12 cents each
• 4 ”x 4” Swatch book: $3.00 each
